TCAA is grateful to have received a $78,320 Mercy C.A.R.E.S. grant to expand health and wellness services for older adults experiencing homelessness through our I-HELP emergency shelter program.
From April 2025 through March 2026, the funding supported 30 seniors by providing enhanced health screenings, access to on-site mental health professionals, educational wellness classes, and additional support to help participants manage chronic health conditions while working toward stable housing. The grant also funded the Claris tablet program, helping 10 housing insecure seniors stay connected to healthcare providers and support services.
The impact of this funding was significant:
30 seniors received enhanced health and housing support.
10 seniors participated in the Claris tablet program.
100% of participants exited into permanent housing.
100% reported feeling better supported through TCAA's services.
99% maintained a connection with their primary care provider or established health home.
One participant, 69-year-old Nolen, came to TCAA after experiencing homelessness for nearly a year. Through I-HELP, he received shelter, healthcare connections, case management, and housing navigation. With consistent support from staff, Nolen regained hope and is now preparing to move into permanent housing.
